Patients with peripheral arterial disease (PAD) are a complex population with many co-morbid conditions. A collaborative multidisciplinary team approach is required to create a plan of care that promotes both a timely and appropriate discharge for those patients who require an interventional/surgical procedure. A revised model of nurse practitioner (NP) practice, a team commitment, a collaborative/integrative partnership (interventional radiology/vascular lab/vascular surgery) and a Heart and Vascular Program objective to improve patient flow were factors that contributed to this focus in care. Identification of stakeholders and of perceived barriers to discharge is an important consideration. Communication, documentation and coordination are the main components of this initiative.
